PERSONAL TRAINER/CLIENT AGREEMENT

  • I, Jay Oppenheim (the “personal trainer”) agree to provide and do the following:

    1. Provide appropriate fitness training and advice.
    2. Provide a program that makes every effort to assure your safety.
    3. Listen to your feedback, wants, and needs, and make adjustment to your training when appropriate.

    I, (the “client”) agree to:

    1. Communicate goals and needs.
    2. Follow the program guidelines during training sessions and on unsupervised training days.
    3. Communicate with the personal trainer during and after the training sessions regarding perceived exertion levels, pain, and/or discomfort.
    4. Communicate satisfaction level with training program and trainer.
    5. Pay the trainer in accordance with this agreement.

    Personal Training Terms and Conditions:

    1. Personal training session(s) that are not rescheduled or canceled 24 hours in advance will result in forfeiting the session(s) and a loss of the value of that/those session(s).
    2. The expiration policy requires completion of personal training sessions within the designated package time. The expectation is that the client will complete sessions in the allotted amount of time. If a client does not complete the sessions during this time period, then another package will be required for future training. The initial training package starts when payment is delivered and future training periods will commence on the Monday after finished training periods. If the client or trainer will be away for a prolonged time, completion time of a package can be extended at the discretion of the trainer.
    3. Payment is required before the initial session. Continuing clients are required to pay for a new package before or by the last session of the current package.
